Abstract

Nem invazív módszerrel vizsgálatuk a kardiovaszkuláris rizikófaktorokat veseelégtelen (CRF) és transzplantált (Tx) gyermekekben. Az autonóm idegrendszeri diszfunkciót (AID) a szívfrekvencia variabilitással (HRV), az érfalmerevséget a pulzushullám terjedési sebességgel (PWV) jellemeztük. Megállapítottuk, hogy AID már gyermekkorban megfigyelhető és elsősorban a fokozott szimpatikotónussal függ össze. Ez Tx után reverzibilis. A PWV mérésénél megállapítottuk, hogy gyermekkori normálértékek meghatározására van szükség. CRF-ben az érfalmerevség fokozott, ez a kalcium, foszfát, és csontanyagcsere zavarával illetve a dialízisben eltöltött idővel függ össze Génpolymorfizmusok. Vizsgáltuk a hősokk-proteinek (HSP) és a toll-like receptorok (TLR) hatását gyermekkori rekurráló húgyúti fertőzésekre (UTI) illetve Tx utáni grafttúlélésre. Megállapítottuk, hogy HSPA1B 1267G és a TLR4 896G allélek UTI-ra hajlamosítanak, függetlenül az egyéb rizikótényezőktől. Ez a terápiás stratégia (tartós kemoprofilaxis) újragondolására késztet. A TLR4 299G allél kapcsolata a hosszú távú grafttúléléssel arra utal, hogy csökkent a proinflammatorikus válasz, ami akut rejekció elleni védelemhez vezet. A HSP72 1267G jelenléte a citoprotektív funkció és a graft túlélés közötti kapcsolatra utal. Patkénymodellen postischemiás akut veseelégtelenségben vizsgáltuk a Na/K ATP-áz működését és lokalizációját. Jelentős különbség volt kimutatható a nőstények javára, amit a HSP rendszer eltérő működése okoz. | Autonomic nervous dysfunction (AND) and arterial stiffness (AS) are independent cardiovascular risk factors in chronic renal failure (CRF). They were studied in children with CRF and following transplantation (Tx) by means of heart rate variability (HRV) and pulse wave velocity (PWV) respectively. AND was already present in children with CRF, it could be attributed to sympathetic overactivity and proved to be reversible following Tx. We established pediatric normal values for PWV. PWV was increased in CRF in connection with the disturbed calcium-, phosphate-, and bone metabolism and the time spent on dialysis. Gene polymorphisms. We studied the role of the heat shock proteins (HSP) and the toll like receptors (TLR) on recurrent urinary tract infections (UTI) and on long-term graft survival following Tx. The presence of the alleles HSPA1B 1267G and TLR4 896G increase the risk of recurrent UTI independently of other factors, suggesting the need to reconsider the actual therapeutic strategies (chemoprophylaxis). TLR4 299G was coupled with long term graft survival, indicating a decreased proimflammatory response and protection against acute rejection. The presence of HSP72 1267G is cytoprotective and increases graft survival. The function and emplacement of the renal tubular Na/ATP-ase pump was characterized in a rat model. A striking gender difference was found favoring females due to the difference of the HSP system.
