Abstract

A kardiális akciós potenciál és az annak hátterében lévő ionáramok regionális eltérésekeit vizsgáltuk kutya és egészséges humán szivekből izolált szívizomsejteken patch-clamp és Western-blott technikával, melynek során az alábbi fontosabb különbségeket kaptuk. 1. Az apicalis akciós potenciálok időtartama rövidebb, korai repolarizációjuk kifejezettebb, mint a basalisoké. Ennek hátterében két káliumáram: az Ito és az IKs, valamint az ezeket mediáló ioncsatornák fokozott apicalis expressziója áll. 2. A midmyocardiális sejtekről elvezetett akciós potenciálok amplitúdója, időtartama és maximális depolarizációs sebessége nagyobb, míg korai repolarizációjuk amplitúdója kisebb, mint az epikardiális sejtekre jellemző értékek. Epicardiális sejteken az Ito és az Iks erősebb, míg midmyocardiális sejteken a nátriumáramért felelős ioncsatornák expressziója dominál. 3. Akciós potenciál clamp technikával jelentős transzmurális különbségeket tártunk fel a kalciumáram lefutásában: epicardiális sejteken a kalciumáram két egymást követő aktivációs csúcsot mutat, míg endocardiális sejtek esetében az áram egyetlen maximummal jellemezhető. Kimutattuk, hogy epicardiális sejteken a már inaktiválódott ill. deaktiválódott kalcium-csatornák újranyílnak, amelynek oka az epicardiális sejtre jellemző spike-and-dome akciós potenciál konfiguráció. | Inhomogeneous distribution of the ventricular action potential and the underlying ion currents and ion channel proteins was studied in canine and human ventricular myocardium using patch clamp and Western blot techniques. The following regional differences were obtained. 1. Myocytes of apical origin had shorter action potentials with larger phase-1 repolarization than the basal ones due to the more pronounced expression of two potassium currents (Ito and IKs) in the epicardial region. 2. Duration, amplitude and maximum rate of depolarization was larger, but phase-1 repolarization was weaker in midmyocardial than in epicardial myocytes. Accordingly, expression of Ito and IKs was weaker, but density of the sodium current was greater in midmyocardial than epicardial cells. 3. Marked transmural differences in the kinetics of the L-type calcium current were explored using action potential clamp techniques. Densities and kinetics of the channels are similar in the two cell types, the twin-peaked appearance of epicardial calcium current is due to the reopening of the deactivated and inactivated calcium channels during the incisura of epicardial action potential.
