Abstract

A TNF-alpha, az IL-8, a TGF-beta és az IL-10 termelését befolyásoló gének un. egy nukleotidos polimorfizmusát (SNP) vizsgáltuk azon betegségekben, melyekben e cytokinek jelentőségét feltételezzük a pathogenezisben.Vizsgálatainkat kiegészítettük a hő-sokk protein- Hsp-70, valamint a szignalizációban fontosnak ítélt CD14, TLR4 és NOD1 receptorokkal kapcsolatos SNP meghatározásokkal. H.pylori pozitív gastritises és ulcusos betegek esetében szignifikáns összefüggést állapítottunk meg az IL-8 -251 T/A SNP, valamint a NOD1 E277K SNP és a betegségre való hajlam között, ugyanakkor a TNF-α, CD14 és TLR4 polimorfizmusokat illetően a genotipusok megoszlása nem tért el az egészséges kontrollokétól. Pancreatitises betegek vizsgálata során a TNF-alpha -308 és az IL-8 promoter polimorfizmus valamint Hsp70-2 SNP tekintetében találtunk összefüggést a pancreatitis súlyosságával. Crohn betegségben, 133 beteg vizsgálata során nem mutattunk ki korrelációt az IL-10 polimorfizmus és a betegség megléte, vagy akár annak súlyossága között, viszont a Hsp70- 2 polimorfizmusnak volt relevanciája. Kimutattuk továbbá, hogy a magas TGF-β termeléssel járó TT genotipus vizsgálataink szerint közel 7-szeresre emeli a bi-vagy pancytopenia valószínűségét myelodysplasztikus syndromában . Felhívtuk a figyelmet a TNF-α genetikai polimorfizmus prognosztikai jelentőségére anti-TNF (Infliximab) kezelésben részesülő betegek esetén. | We aimed to determine whether single nucleotid polymorphisms (SNPs) associated with differential expression of TNF-alpha, IL-8, TGF-beta and IL-10 are associated with susceptibility to different gastrointestinal or haematologic diseases. In addition, SNPs of Hsp-70, and SNPs of CD14, TLR4 and NOD1 receptors were also investigated. Genetic polymorphism of interleukin-8 (IL-8) and NOD1 and IL-8, but not polymorphisms of TLR4 genes, were associated with Helicobacter pylori-induced duodenal ulcer and gastritis. Polymorphism of the TNF-α, HSP70-2, and CD14 genes were involved in increased susceptibility to severe acute pancreatitis. In addition, polymorphism in the IL-8 gene, but not in the TLR4 genes were connected with the severity of acute pancreatitis. Investigation of SNPs in 133cases with Crohn?s disease revealed, that the polymorphism of heat-shock protein gene HSP70-2, but not the polymorphisms of the IL-10 and CD14 genes, were associated with the outcome of Crohn's disease. Clinical importance of transforming growth factor-beta rather than the tumor necrosis factor-alpha gene polymorphisms was observed in patients with the myelodisplastic syndrome belonging to the refractory anaemia subtype. Finally, the prognostic value of TNF-alpha -301 gene polymorphism among patients treated with anti-TNFα –Infliximab was investigated, and a connection of TNF/2 allele- with high TNF producing capacity- and therapy-refractory cases was proved.
