Abstract

A petefészekrákok második vonalú kemoterápiáival 2,3–42 hónapos progressziómentes túlélésekről lehet olvasni. Célkitűzés: A szerzők a második vonalú paclitaxel-carboplatin-, topotecan-, és cisplatin-epirubicin-etoposid kemoterápiákat értékelték. Módszer: A paclitaxel-carboplatin kombinációt 13 beteg 3 hetente 175 mg/m <sup>2</sup> , illetőleg AUC 5-dózisban, a topotecant 16 beteg 3 hetente, 1,5 mg/m <sup>2</sup> /1–5 nap dózisban, a cisplatin-epirubicin-etoposid kombinációt (33 mg/m <sup>2</sup> /1–3 nap – 60 mg/m <sup>2</sup> /1 nap – 100 mg/m <sup>2</sup> /1–3 nap) pedig 4 hetenként 48 beteg kapta. A progressziómentes túlélés statisztikai értékelése a Kaplan–Meier-féle product-limit eljárás szerint történt. Eredmények: A paclitaxel-carboplatin és a cisplatin-epirubicin-etoposid kemoterápiákkal a topotecanhoz képest szignifikánsan jobb progressziómentes időszak (5 és 5,5 hónap vs 4 hónap, p = 0,0324 és p = 0,0087) igazolódott, és mindhárom kezelés jobb progressziómentes időszakot (6 és 3,5 hónap, 7 és 4 hónap ill. 6,5 és 3,5 hónap) eredményezett a platinaérzékeny betegekben, mint a platinarezisztensekben. Következtetés: Az alacsonyabb árú cisplatin-epirubicin-etoposid kombinációval a másik 2 kezeléshez képest valamivel jobb eredmény figyelhető meg platinaérzékeny és platinarezisztens daganatokban is. | Introduction: Second line chemotherapies result in a progression free-interval of 2,3–42 months. Aim: Authors evaluated the efficacy of second line paclitaxel-carboplatin, topotecan, and cisplatin-epirubicin-etoposide therapies. Methods: Paclitaxel-carboplatin (175 mg/m2 and AUC 5) in 3-week intervals and topotecan (1,5 mg/m <sup>2</sup> /d1–3) in 3-week intervals was given to 13 and 16 patients. The triplet combination of cisplatin-epirubicin-etoposide was applied in 48 patients in 4-week courses, with the doses of 33 mg/m <sup>2</sup> /d1–3, 60 mg/m <sup>2</sup> /d1, and 100 mg/m <sup>2</sup> /d1–3., respectively. The progression-free interval was calculated by the product limit method of Kaplan–Meier. Results: A significantly better progression-free interval was observed in treatments with paclitaxel-carboplatin and cisplatin-epirubicin-etoposide compared to the topotecan chemotherapy (5 and 5,5 months vs. 4 months, respectively, p = 0.0324 and p = 0.0087). A better progression-free interval was found in the platinum-sensitive tumors compared to the platinum-resistant ones by the chemotherapies mentioned above (6 and 3,5 months, 7 and 4 months, and 6,5 and 3,5 months, respectively). Conclusion: The lower priced cisplatin-epirubicin-etoposide combination resulted in a slightly better efficacy compared to the 2 other treatments both in platinum-sensitive and platinum-resistant tumors.
