Abstract

Kutatómunkánk során klinikai, illetve experimentális elektrofiziológiai vizsgálatok segítségével a kamrai tachyarrhythmiák kialakulását befolyásoló tényezők jelentőségét vizsgáltuk, továbbá a klinikai gyakorlatban hasznosítható diagnosztikus és terápiás lehetőségeket keresünk. In vivo kísérletek során jellemeztük az intracoronariasan adott endothelin-1 elektrofiziológiai hatásait. Szelektív ET-A antagonista alkalmazásával vizsgáltuk az antiarrhythmiás hatást ischaemiás modellben. Az ET-1 és szimpatikus aktiváció együttes hatását elsőként vizsgáltuk. Klinikai vizsgálatok során vizsgáltuk az ET-1 szerepét a spontán ritmuszavarok keletkezésében.Klinikai vizsgálatan igazoltuk a csökkent MIBG felvétel jelentőségét a malignus kamrai tachyarrhythmiák keletkezésében.Az AV csomót beidegző paraszimpatikus plexus szelektív stimulációjával hatékonyan csökkentettük a kamrai frekvenciát mind experimentális, mind klinikai körülmények között. Experimentális és klinikai vizsgálatokban a resynchronizatios terápia hatékonyságát vizsgáltuk. Új módszereket dolgoztunk ki a hatékonyság fokozására.ICD-s betegeknél spontán kamrai ritmuszavarok jellegzetességeit vizsgáltuk, illetve detekciós algoritmusokat dolgoztunk ki a diszkriminációs hatékonyság fokozására. A kamrafibrilláció indukciójára alkalmazott módszerek hatékonysága között klinikailag jelentős különbség nem volt igazolható. | We have examined the factors affecting the evolution of ventricular tachyarrhythmias during clinical and experimental investigations and searched for useable diagnostic and therapeutic opportunities in the clinical practice.During in vivo experiments we described the electrophysiological effects of endothelin-1 given intracoronary. We examined the antiarrhythmic effects of a selective ET-A antagonist in an ischemic model. We were the first to examine the effects of ET-1 and sympathetic activation simultaneously. We performed clinical studies to determine the role of ET-1 in the evolution of spontaneous arrhythmia.The significance of decreased MIBG uptake was proven in the pathmomechanism of ventricular tachyarrhythmia.The ventricular rate was effectively reduced during atrial fibrillation with the selective stimulation of the parasympathetic node innervating the AV node during experimental and clinical conditions.We examined the efficacy of cardiac resynchronization therapy in experimental and clinical studies. New methods have been developed to increase the efficacy. Spontaneous arrhythmias of ICD patients were studied. New detection algorythms were developed to increase the discrimination efficacy. No clinically significant difference could be proven in the efficacy of the methods of ventricular fibrillation induction.
