Abstract

2003-2006. között 31 kiválasztott garat- és gégedaganatos beteget vizsgáltunk meg a photodynámiás effektuson alapuló 5-aminolevulinsav (ALA) által indukált protoporphyrin IX (PPIX) fluoreszcencia módszerével, majd további 59 betegnél az autofluoreszcencia (DAFE) módszerét alkalmaztuk a daganatok határainak, felszíni kiterjedésének pontosabb megítélésére és a módszerek összehasonlítása céljából. A vizsgálatok során a értékeltük a daganatok által mutatott fluoreszcencia intenzitását. Mind az ALA fluoreszcencia, mind az autofluoreszcencia intenzitását a garatív-tonsilla és a hangszalagtumoroknál találtuk a legmagasabbnak, így jól látható fluoreszcenciát kaptunk, mely jól elkülönült az ép szövetektől. A módszerek szenzitivitása 95% feletti, a specificitása 60% körüli, mert a dysplasiás (daganatmegelőző állapotok) elváltozások nem mindig különíthetők jól el a már daganatszövettől. A tonsilla-garatívtumorok és hangszalagtumorok esetében a daganatszövet kiterjedési viszonyainak megítélését jelentősen javítja az eljárás, több betegnél ez a módszer biztosította a felszíni tumor határának elkülönítését. A hangszalagtumorok transorális laser rezekciója (chordectomia) előtti és a mesopharynx tumorok diagnosztikáját segítette mindkét módszer, de az autofluoreszcencia alkalmazása egyszerűbb. | In a period of 2003-2005, 31 patients with selected pharyngo-laryngael tumours underwent induced photodynamic imaging, 5-aminolevulinic acid (ALA) induced protoporphyrin IX (PPIX) fluorescence, and in 59 patients with the same type of tumours autofluorescence method was applied for the assessment of outlining, extension of tumours and comparison of these methods. Intensity of red fluorescence was measured. Autofluorescence has an advantage, that induction substance does not need to be applied. Mesopharynx and laryngeal (vocal cord) tumours can be well differentiated from healthy tissue examined by both method. For the preoperatve diagnostic before transoral laser resection (chordectomy) tumours, wound (line of the resection) and specimens were assessed, that can help the planning of the surgery and resection of the tumour. Fluorescence intensity of ALA fluorescence and autofluorescence image was highest in tumours of the tonsil-soft palate and vocal cord, showing clear delineation from healty tissue. Sensitivity of the methods is over 95%, specificity about 60%, because dysplastic lesions were not clearly differentiated by intensity. In mesopharyngeal and vocal cord tumours ALA fluorescence and autofluorescence imaging methods improved the assessment of superficial extension of tumours, helped the outlining of tumours and the preoerative dianostic of transoral laser resection (chordectomy), but the application of autofluorescence technique is simpler.
