Abstract

A sok éve ismert és inzulinnal is kezelt 2. típusú diabeteses beteg paraesthesiáit ismétel- ten diabeteses neuropathia következményének tartották. A paraesthesiák felső végtagokon észlelhető domináns jelentkezése, a fájdalmas izomgörcsök jelenléte és a strumectomiás előzmény alapján késői fellépésű posztope- ratív hypoparathyreosis volt igazolható. Tetania nem volt jelen. Kalciumpótlással a tünetek teljesen megszűntek. Irodalmi adatok szerint a késői fellépésű posztoperatív hypoparathyreosis dominálóan neuropszichiátriai tüne- tekkel jelentkezik, de a hypocalcaemia kardiális tünetei is megfigyelhetők. A nyaki sebészeti előzmény – tekintet nélkül a műtét óta eltelt időre – megkönnyítheti a hypocalcaemia szerteágazó tüneteinek felismerését. | Paresthesia’s at a patient with long-standing type 2 diabetes mellitus were assumed to be conse- quences of diabetic neuropathy. As the paresthesia’s touched both the upper and lower extremities with painful muscle cramps and based on previous strumectomy a late onset postsurgical hypoparathyroidism was proved. Tetany was not present. Although there were signs of the presence of diabetic neuropathy too, calcium supple- mentation eliminated the symptoms. Literal data have shown that late onset postsurgical hypoparathyroidism presents dominantly with neuropsychiatric symptoms but cases with other hypocalcemic symptoms – e.g., car- diomyopathy – have also been observed. The history of previous neck surgery can help recognizing the atypical signs of hypocalcemia.
