Abstract

Az előrehaladott és metasztatikus prosztatarák kezelésének alapja az androgéndeprivációs terápia, amely során a here androgén szekréciójának különböző módon történő elnyomását végezzük. Leggyakrabban használt formája a kémiai kasztráció LHRH analóg/antagonista készítményekkel. Lokálisan előrehaladott prosztatarák esetén a hormonterápia (ADT) kombinációja javasolt helyi kezeléssel (radioterápia vagy radikális prostatectomia) a túlélés javítása érdekében. Radikális műtét után igazolt nyirokcsomó-pozitivitás esetén a korai adjuváns hormonterápia javítja a daganatspecifikus túlélést, amelynek kiegészítése a prosztataágy sugárkezelésével további előnyt jelenthet. Metasztatikus prosztatarák esetén is a kombinációs kezelések jelentik a standard elsővonalbeli terápiát, elsősorban ADT és ARPI (abirateron, apalutamid, enzalutamid) együttes adásával. = The basis of the treatment of advanced and metastatic prostate cancer is androgen deprivation therapy, where the androgen secretion of testis is suppressed in different ways. The most frequently used form is chemical castration with LHRH agonist/ antagonist injections. For locally advanced prostate cancer a combination of hormone therapy (ADT) with local treatment (radiotherapy or radical prostatectomy) is recommended to improve survival. In case of confirmed lymph node positivity after radical surgery early adjuvant hormone therapy improves tumour-specific survival, which can be combined with irradiation of the prostate bed as an additional benefit. Even in metastatic prostate cancer combination treatments are the standard first-line therapies, primarily with the combined administration of ADT and ARPI (abiraterone, apalutamide, enzalutamide).
