Abstract

A kézirat a DAPA-CKD vizsgálat kapcsán áttekinti a nátrium-glükóz kotranszporter (SGLT)-2-gátlók kedvező vesehatásainak biokémiai mechanizmusait és a nefroprotektív hatásukat alátámasztó klinikai tanulmányokat. Részletesen foglalkozik a vizsgálat előzményeivel és eredményeivel. Rámutat, hogy a krónikus vesebetegség patogenezise diabetesben és attól függetlenül SGLT-2- (és valószínűsíthetően nátrium-hidrogén exchanger-3) függő mechanizmusú. E tényezők aktivitásának csökkentése/gátlása új terápiás opciót jelent a vesekárosodás megelőzésében, de legalább folyamatai lassításában. Az SGLT-2-gátlók bevezetése a cukorbetegség jelenlététől függetlenül új korszakot nyithat a krónikus vesebetegség terápiájában. | The manuscript in context with the DAPA-CKD study reviews the biochemical mechanisms of the beneficial renal effects of sodium glucose cotransporter (SGLT)-2 inhibitors and the clinical studies supporting their nephroprotective effect. It deals in detail with the background and results of the study and its forerunners. It is pointed out that the pathogenesis of chronic kidney disease in diabetes and other states seems to be SGLT-2 (and possibly sodium hydrogen exchanger-3)-dependent. Reducing/inhibiting the activity of these factors represents a new therapeutic option in preventing the kidney damage, but at least in slowing down its processes. The introduction of SGLT-2 inhibitors irrespective of the presence of diabetes may open a new era in the therapy of chronic kidney disease.
