Abstract

Napjaink kezelési irányelveiben az új vércukorcsökkentő gyógyszercsoportok szívelégtelenséget, krónikus vese- és/vagy atherosclerotikus szívbetegséget előnyösen befolyásoló hatásainak ismeretében átrendeződtek a terápiaválasztás szempontjai. Felmerül a kérdés, hol a helye a már régóta alkalmazott gyógyszercsoportoknak, azon belül a szulfanilureáknak és e származékok között, igazolt előnyei ismeretében, a gliklazidnak. Ennek megválaszolását tűzte ki célul az áttekintő közlemény. Sorra véve a készítménycsoport hatékonyságát, a koronavírus-járvány idején is megfigyelt biztonságosságát megállapítja, hogy még jó ideig a terápiás eszköztár része marad, s a gliklazid is őrzi kiemelt helyét a csoporton belül. Alacsony hipoglikémia-kockázatát ramadán idején végzett megfigyelések is megerősítették, obszervációs vizsgálatban a szitagliptinnél hatékonyabbnak és azonosan biztonságosnak bizonyult. A nátrium-glükóz kotranszporter-2 gátlókkal való kombinációjuk a vércukorcsökkentő kezelés hatékony alternatívája lehet. | In today's treatment guidelines, recognizing the beneficial effects of the new blood glucose lowering drugs on heart failure, chronic kidney disease, and/or atherosclerotic heart disease main aspects of the choice of drugs have been reorganized. In light of this arises the question, where is the present therapeutic place of already long-time used antihyperglycemic compounds, like the sulphonylureas, and within the class, of the drug with proven individual benefits, the gliclazide. To answer this question is the aim of the review paper. Taking into consideration the efficacy and the safety of the group, observed even among circumstances of the coronavirus epidemic, it can be concluded, that it will remain part of the therapeutic toolbox also in the near future. Also gliclazide continues to play a key role in the group. T his is further supported with the low hypoglycaemia risk confirmed by observations during Ramadan as well as its proven equal safety and greater effectivity as compared to sitagliptin in a recent observational study. Its combination with sodium glucose cotransporter-2 inhibitors may be an effective new alternative to blood glucose lowering therapy.
