Abstract

Bevezetés: A palliatív ellátás célja a krónikus, progresszív betegségben szenvedő, súlyos állapotú betegek életminőségének javítása. Munkánk során a krónikus hemodialízis (HD)-kezelésben részesülő betegek tüneteit, állapotát és életvégi kívánságát mértük fel annak megítélésére, hogy palliatív ellátásuk indokolt lenne-e. Betegek és módszerek: Munkánk első részében 2022. július 1. és 2023. december 31. között a Pécsi TritonLife Dialízis Centrumban és Szatellita Állomáson a krónikus HDkezelt, előrehaladott daganatos betegségük miatt palliált betegeket két csoportba osztva vizsgáltuk tüneti terheik és életvégi kívánságuk szempontjából. Mindkét csoportban (A és B) a betegek (n=5 mindkét csoportban) tüneti terheit hetente mértük az Edmonton Symptom Assessment System Revised Renal (ESAS-r: Renal) kérdőív segítségével, a B csoport életvégi kívánságát havonta rögzítettük. Munkánk második részében a krónikus HD-kezelteket (n=168) vizsgáltuk tüneteik (Integrated Palliative Care Outcome Scale Renal [IPOS-Renal] kérdőív), többszervi érintettségük (Charlson komorbiditási index) és funkcionális állapotuk (” meglepődéskérdés”) szempontjából. Eredmények: Első vizsgálatunkban a tüneti terhek felmérése nem volt sikeres, a betegek nem tudták értékelni a skálán tüneteik időbeli változását. A halál bekövetkeztének helyére vonatkozó életvégi kívánság a B csoportban mindkét elhunyt beteg esetében teljesült. Második vizsgálatunkban a krónikus HD-kezeltek jelentős totális fájdalmára derült fény. A Charlson komorbiditási index magas volt (6±2), amely 98%-os tízéves és a ” meglepődéskérdés”-re adott 46 negatív válasz 27%-os egy éven belüli várható halálozást jelent. Megbeszélés: A krónikus HD-kezeltek magas tüneti terhei és várható halálozása palliatív kezelésük szükségességét hangsúlyozza. | Introduction: Palliative care improves the quality of life in patients with serious condition suffering from chronic, progressive disease. In our work, we assessed the symptoms, condition and end-of-life wishes of patients treated with chronic haemodialysis (HD) in order to assess whether palliative care would be justified. Patients and methods: In the first part of our work, between 1 July, 2022 and 31 December, 2023 in the TritonLife Dialysis Center and Szatellita Station in Pécs, chronic HD treated patients with advanced cancer divided in two groups were examined regarding their symptoms and the end-of-life wishes. The symptoms in both groups (A and B) (n=5 in both groups) was assessed weekly using the Edmonton Symptom Assessment System Revised Renal (ESAS-r: Renal) questionnaire and the end-of-life wishes of patients in group B were recorded monthly. In the second part of our work, we examined the chronic HD patients (n=168) concerning their symptoms (Integrated Palliative Care Outcome Scale Renal /IPOS-Renal/ questionnaire), their multiorgan involvement (Charlson comorbidity index) and their functional status (“surprise question”). Results: In the first part of our work, the assessment of symptoms using this questionnaire was not successful. The end-of-life wishes for the place of death could be fulfilled in both dead patients in group B. In the second part of our work, in chronic HD treated patients, a significant symptomatic burden was revealed. The Charlson comorbidity index was high (6±2), which means 98% expected mortality within the following ten years, and the 46 negative answers to the “surprise question” mean 27% expected mortality within the next year. Discussion: The high burden of symptoms and expected mortality of chronic HD patients clearly suggest the need for their palliative care.
