Abstract

Az odontogen sinusitis az orrmelléküreg-gyulladások szekunder izolált formája, amelynek hátterében odontogen gócos folyamat, idegen test és/vagy a Schneider-membránt érintő korábbi beavatkozás állapítható meg. A rhinosinusitist a gátolt orrlégzés, elülső vagy garati váladékcsorgás, feszítés/fájdalomérzés és szaglásvesztés tünetsorból legalább kettő jelenléte és orrendoszkópos eltérések jellemzik. Az akut forma infektológiai kórkép, a krónikus viszont immunpatológiai jellegű, primer diffúz kétoldali betegség. Az odontogen sinusitis, illetve az akut és a krónikus rhinosinusitis a tünetekben, a patomechanizmusban és a terápiában jelentősen eltérő kórformák, ezért a pontos diagnózis alapvető fontosságú. Az odontogén sinusitis megjelenésében az akut bakteriális rhinosinusitishez hasonló, a bűzös orrfolyás és/vagy az orrból eredő foetor jellegzetes lehet, illetve a szaglásvesztés nem jellemző. Típusos klinikai tünetek és endoszkópos lelet nélkül az arckoponya-CT-n észlelt nyálkahártya-elváltozások önmagukban nem elegendőek az odontogen sinusitis diagnózisának felállításához. A terápia célja minden esetben az akut panaszok kezelése, a gyulladás megszüntetése, a megfelelő arcüregi drenázs biztosítása, a góc szanálása, az esetleges idegen test eltávolítása, illetve az oroantralis fistula zárása. Az első három célpont tekintetében a konzervatív gyógyszeres terápiát részesítjük előnyben, amennyiben ez nem vezet eredményre, akkor az endoszkópos sinussebészeti beavatkozások következnek. Az egyedi és/vagy komplikált esetekben lényeges szempont a fogorvosi/szájsebészeti/fül-orr-gégészeti konzílium, valamint az ismételt CT és endoszkópos vizsgálat. A döntéshozatalban diagnosztikus és terápiás algoritmusok lehetnek segítségünkre. | Odontogenic sinusitis is classified as a localized, secondary form of the paranasal sinus inflammation, in which odon- togenic focus, foreign body and/or any surgical intervention violating the Schneiderian membrane may be found in the background. Rhinosinusitis is defined by at least two from the symptoms of nasal blockage, anterior or posterior nasal discharge, facial discomfort/pain, loss of smell and typical alterations with nasal endoscopy. Acute rhinosinusitis seems to be infectious, while the chronic phenotype shows immunopathological signs of primary, diffuse bilateral disease. Acute and chronic rhinosinusitis as well as odontogenic sinusitis appear to have markedly different symptoms, pathomechanistic properties and therapeutic possibilities, thus the appropriate diagnosis is of utmost importance. The symptoms of the odontogenic sinusitis is similar compared to acute bacterial rhinosinusitis, though putrid nasal discharge and/or foetor from the nose might be pathognomic, while anosmia is usually not present. Mucosal chang- es and alterations in sinus-CT scans without typical clinical symptoms and nasal endoscopic findings do not define the diagnosis of odontogenic sinusitis. The goal of the therapy comprises treatment of the acute symptoms and inflam- mation, improvement of sinus drainage, eradication of the odontogenic focus, removal of foreign bodies if present, and finally closure of the oroantral fistules, if needed. Conservative medical therapy should be initiated first, and in the case of failure, endoscopic sinus surgical intervention is considered as the next step. In individual and more com- plicated cases, consultation with dental-, maxillo-facial surgeon and specialist in otorhinolaryngology is recommend- ed, together with repeated nasal endoscopy and sinus-CT-scan. Diagnostic and therapeutic algorithms are suggested in order to promote up-to-date treatment of odontogenic sinusitis patients.
