Abstract

A primer aldosteronismus a leggyakoribb endokrin hypertoniaforma, az összes eset körülbelül 5-20%-a. Egy fiatal, obes nőbeteget először 2014-ben vizsgáltunk ki rezisztens hypertonia miatt, de akkor szekunder okot nem találtunk az elhízáson kívül. 2023-ban vérnyomása megemelkedett, hypokalaemia jelent meg. Spironolaktonra vérnyomása szignifikánsan javult, a szérumaldoszteron-koncentráció megemelkedett, a CT jobb oldali mellékvese-adenomát írt le. 2024 nyarán sikeres laparoszkópos adrenalectomia történt, utána vérnyomása monoterápiával céltartományban mozgott. Primer aldosteronismus gyanúja esetén változatlan terápia mellett is el lehet és el kell végezni az egyszerű szűrővizsgálatokat! | Primary aldosteronism is the most common form of endocrine hypertension, accounting for approximately 5-20% of all cases. A young, obese female patient was first evaluated in 2014 for resistant hypertension, but at that time no secondary cause was identified apart from obesity. In 2023, her blood pressure increased, and hypokalemia developed. Her blood pressure significantly improved with spironolactone, her serum aldosterone concentration increased, and a CT scan revealed a right adrenal adenoma. In the summer of 2024, a successful laparoscopic adrenalectomy was performed, after which her blood pressure remained within the target range with monotherapy. In cases of suspected primary aldosteronism, simple screening tests can and should be performed even while maintaining the current therapy!
