Abstract

Aims: Assessment of circadian blood pressure variability in people with impaired glucose tolerance (IGT). Methods: 75 people with IGT and 40 healthy volunteers underwent detailed neurological assessment. Autonomic neuropathy was detected by the five standard tests of cardiovascular autonomic function and heart rate variability characterised by the triangle index (HRVti). Ambulatory blood pressure parameters (means values±SD, diurnal indices) were assessed by 24-hour ambulatory blood pressure monitoring. The systolic and diastolic diurnal indices were defined as follows [(mean value during the day minus mean value during the nighttime) x 100] / mean value during the day. Results: Both systolic and diastolic diurnal indices were significantly lower in IGT people compared to controls. Mean 24-hour systolic and diastolic blood pressure values were significant higher in the IGT group versus controls. There were significant correlations between decreased beat-to-beat variation and diminished 24-hour blood pressure measurement. Conclusion: Our data suggest that cardiovascular autonomic neuropathy is associated with diminished circadian blood pressure changes in people with IGT.
