Abstract

The discovery and implementation of fluoride in preventing dental decay marked one of the most significant public health advancements of the 20th century. Observations of stained teeth, initially described as "black teeth," date back to Giulio Cesare Capaccio in 1634, with later scientific documentation by J.M. Eager in 1901. However, it was Frederick McKay’s investigations in Colorado Springs that led to the recognition of fluoride’s role in both staining and protecting teeth from decay. His collaboration with G.V. Black and later research by H.V. Churchill confirmed that high fluoride levels in drinking water caused fluorosis but also resulted in lower rates of dental caries. The work of H. Trendley Dean at the National Institute of Health established an optimal fluoride concentration (0.7–1.2 ppm) to maximise cavity prevention while minimising fluorosis. Fluoridation trials in Grand Rapids, Michigan (1945), followed by studies in Canada, the UK, and New Zealand, demonstrated significant reductions in cavities. By the 1960s, fluoridation became a global public health policy, endorsed by major health organisations. Despite opposition and conspiracy theories, scientific evidence continues to affirm fluoride’s safety and effectiveness in preventing tooth decay, benefiting millions worldwide.
