Abstract

szerzők (Észak-Pesti Centrumkórház – Honvédkórház Fül-orr-gége Osztály, Fej- és Nyaksebészeti Osztály) egy öngyilkossági kísérletről számolnak be, melynek során egy 48 éves férfi légpuskával – a fejtetőt célozva – a szájába lőtt. A lövedék áthatolt a kemény szájpadon és bejutott az orrba, végül az elülső rostasejtekben kötött ki, a koponyaalaphoz közel. A CT felvételek áttanulmányozását követően endoszkópos feltárás mellett döntöttek. Hagyományos endoszkópos ethmoidectomiát végeztek, ennek során az apró lövedéket könnyen megtalálták és eltávolították. A pszichiátriai konzílium további önpusztító szándékot nem igazolt, ezért jó fizikai és mentális állapotban hazaengedték a beteget. Bár a légpuskatöltények sebessége a törvényes határértékeken belül van, jelen eset azt mutatja, hogy az ilyen fajta lövedék is képes vastag csontokon áthatolni és érzékeny struktúrákat sérteni. = A case of attempted suicide is reported where a 48-year-old man was shot in the mouth, pointing the top of his head with an air rifle. The projectile penetrated the hard palate and got into the nose, finally lodging in the anterior ethmoid cells close to the anterior skull base. After analyzing the CT scans, an endoscopic approach was decided to remove the pellet. A conventional endoscopic ethmoidectomy was performed during which the small bullet was easily found and removed. An examination by a psychiatrist revealed no intention for further self-destruction. The patient was finally discharged from the hospital in good physical and mental condition, without any severe symptoms. Although the projectile’s velocity from an air rifle is within legal limits, the present case demonstrates its potential to perforate rather thick bones and endanger vulnerable structures.
