Abstract

A technécium-99m-MIBI (metoxi-izobutil-izonitril) egy radiofarmakológiai készítmény, amelyet a szív vérellátásának, illetve mellékpajzsmirigy-szövetek patológiájának kivizsgálására használnak. A MIBI-vel végzett szcintigráfia gyakran használt diagnosztikus módszer mellékpajzsmirigy-adenoma kivizsgálásának részeként az elváltozás lokalizálására, esetleg ectopiás adenoma kizárására, azonban pajzsmirigy-adenoma, pajzsmirigyrák és egyéb sejtdús, sok mitokondriumot tartalmazó elváltozások is okozhatnak izotópdúsítást. Tanulmányunkban egy 79 éves nőbeteg esetét mutatjuk be, aki 1997 óta osteoporosis miatt endokrinológiai kezelés alatt áll. A 2023 végétől intermittáló hypercalcaemia és emelkedett parathormonszintek hátterében primer hyperparathyreosist véleményeztünk. A nyaki ultrahangvizsgálattal és a SPECT/CT-vel kiegészített mellékpajzsmirigy-szcintigráfiás vizsgálat a bal alsó mellékpajzsmirigy-adenomára utaló eltérést igazolt, emellett a bal retro/parapharyngealis térségben is kifejezett izotóphalmozás volt látható, ami a malignitás lehetőségét vetette fel. Az MRI alapján a parapharyngealis laesio malignus jellege nem merült fel. A sebészi feltárás során a bal felső mellékpajzsmirigy-adenoma és a parapharyngealis térben elhelyezkedő képlet eltávolításra került, ez utóbbi minimálinvazív módon, transoralis endoszkópasszisztált technikával. A szövettani vizsgálat mellékpajzsmirigy-adenomát, illetve nyálmirigy-oncocytomát igazolt. Az eset rávilágít a SPECT/CT-vel kiegészített mellékpajzsmirigy-szcintigráfia jelentőségére a mellékpajzsmirigy-adenoma lokalizációs diagnosztikájában, valamint arra, hogy a fokozott izotóphalmozás nem minden esetben mellékpajzsmirigy-eredetű eltérést jelez. Orv Hetil. 2026; 167(9): 351–358. | Technetium-99m-MIBI (methoxy-isobutyl-isonitrile) is a radiopharmaceutical agent used in the evaluation of myo- cardial perfusion and parathyroid pathologies. MIBI scintigraphy is a commonly applied diagnostic method in the assessment of parathyroid adenomas for lesion localization and to exclude the presence of ectopic adenomas. However, tracer uptake may also occur in thyroid adenomas, thyroid carcinomas, and other lesions with high mitochon- drial density. In our study, we present the case of a 79-year-old female patient. The patient had been under endo- crinological care since 1997 due to osteoporosis. Since late 2023, intermittent hypercalcemia accompanied by elevated parathyroid hormone levels has been consistent with a diagnosis of primary hyperparathyroidism. Neck ul- trasound and parathyroid scintigraphy with SPECT/CT confirmed an abnormality suggestive of an adenoma of the left superior parathyroid gland, and there was also an intense isotope accumulation in the left retro/parapharyngeal region, suggesting the possibility of malignancy. Based on MRI, the parapharyngeal lesion was not suspected to be malignant. During surgical exploration, the adenoma of the left lower parathyroid gland and the lesion located in the parapharyngeal space were removed, the latter using a minimally invasive, transoral endoscope-assisted technique. Histology confirmed parathyroid adenoma and salivary gland oncocytoma. This case highlights the importance of parathyroid scintigraphy with SPECT/CT in the localization of parathyroid adenomas and that increased isotope accumulation does not always indicate a parathyroid abnormality.
