Abstract

Az akut epehólyag-gyulladás gold standard terápiája a cholecystectomia, azonban az irányelvek középsúlyos és súlyos gyulladás esetén a műtétre nem alkalmas betegnél is az epehólyag tehermentesítését javasolják a konzervatív terápia mellett. A perkután transhepaticus epehólyagdrenázs mellett az en­doszkópos megoldások (például a transpapillaris és az endoszkópos ultrahang által vezérelt epehólyag­drenázs) is régóta elérhetőek, azonban a klinikai gyakorlatban még kevésbé terjedtek el. A transpapillaris epehólyagdrenázs technikai és klinikai sikeressége összevethető a perkután transhepaticus epehólyag­drenázséval, míg szövődményrátája alacsonyabb, valamint kisebb eséllyel alakul ki ismételten gyulladás a műtétig eltelt időben. | The gold standard therapy to acute cholecystitis is cholecystectomy, although in the cases of moderate and severe inflammation, guidelines recommend drainage with conservative therapy in patients who are unfit for surgery. Transpapillary gallbladder drainage and endosonography-guided gallbladder drainage has been an alternate option for over 20 years to percutaneous transhepatic gallbladder drainage, however, the procedures haven’t been used widespreadly in everyday clinical practice. The technical and clinical success rate of transpapillary gallbladder drainage is comparable with percutaneous transhepatic gallbladder drainage, the rate of adverse events is lower, and there is less chance of a recurring inflammation in the time to surgery
