Abstract
Célkitűzés: A hypertonia és az epilepszia közötti kapcsolategyre hangsúlyosabb kutatási terület. A közlemény célja az agyi renin-angiotenzinrendszer (RAS) szerepének és az angiotenzinreceptor-blokkoló (ARB) gyógyszerekpotenciális neuromoduláló hatásainak összefoglalása epilepsziában. Módszer: A rendelkezésre álló preklinikai és humán vizsgálatok áttekintése alapján összefoglaltuk a központi idegrendszeren található RAS komponenseinekszerepét az epilepszia kialakulásában, különös tekintettel az ARB antikonvulzívés antiepileptogén szerepére. Eredmények: Epilepszia esetén az agyban a RAS diszfunkciója figyelhető meg,amely feltételezhetően fokozza a neuronalis excitabilitást és az epilepsziás hálózatok kialakulását. Az 1-es típusú angiotenzin-II-receptor blokkolása gátoljaa neurotoxikus hatást létrehozó intracelluláris jelátviteli utakat, és következményesen felerősíti a neuroprotektív hatásokat. Az ARB hatóanyagokkal folytatott preklinikai vizsgálatokban antikonvulzív, antiepileptogén és neuroprotektívhatásokat figyeltek meg. Retrospektív obszervációs humán vizsgálatok szerintARB-kezelés mellett alacsonyabb az epilepszia incidenciája más antihipertenzívterápiákhoz képest.Következtetés: Az ARB-kezelés ígéretes lehetőséget jelent az epileptogeneziskontrollálásában, különösen a hypertonia és szövődményeinek, például a cerebrovascularis eseményeket követő epilepsziák esetén. A jelenlegi bizonyítékoktovábbi prospektív és randomizált vizsgálatokat indokolnak annak tisztázásaérdekében, hogy az ARB-kezelés beépíthető-e a jövőbeli hypertonia- és epilepsziakezelési irányelvekbe. | Objective: The relationship between hypertension and epilepsy has emerged as an important area of research. This review summarizes the role of the cerebral renin-angiotensin system (RAS) and explores the potential neuromodulatory effects of angiotensin receptor blockers (ARBs) in the management of epilepsy. Methods: Available preclinical and human studies were reviewed to evaluate the function of brain RAS components, epilepsy-related molecular alterations, and the therapeutic mechanisms and clinical relevance of ARB therapy. Results: Epilepsy is associated with selective hyperactivation of the brain RAS, which increases neuronal excitability and promotes epileptic network formation. Blockade of the angiotensin II type 1 receptor inhibits intracellular signaling pathways that mediate neurotoxic effects, thereby enhancing neuroprotective mechanisms. In preclinical studies, ARBs demonstrate anticonvulsant, antiepileptogenic, and neuroprotective effects. Retrospective observational studies in humans consistently report a lower incidence of epilepsy among patients treated with ARBs compared with those receiving other antihypertensive therapies. Conclusion: ARB treatment may represent a promising strategy to modulate epileptogenesis, particularly in patients with hypertension and related complications, such as post-stroke epilepsy. Current evidence supports the need for prospective, randomized trials to determine whether ARB therapy can be incorporated into future management guidelines for hypertension and epilepsy.
