Abstract

Egy fiatal nőbeteg esete kapcsán mutatjuk be a phaeochromocytoma felismerésének és menedzsmentjének kulcsfontosságú szempontjait.A közlemény fókuszában a fiatalkori súlyos hypertonia és tachycardia hátterében álló szekunder etiológiák differenciáldiagnosztikája, valamint a célzott képalkotó és biokémiai vizsgálatok szerepe áll. Külön hangsúlyt kap a β-blokkoló terápiát követő paradox vérnyomás-emelkedés, mint a rejtett phaeochromocytomakritikus figyelmeztető jele. Ismertetjük az α-adrenerg-blokád elsődlegességét, aperioperatív hemodinamikai optimalizáció jelentőségét és a definitív sebészi intervenció szerepét. Az eset rávilágít arra, hogy a korai felismerés és a helyes terápiás szekvencia elengedhetetlen a súlyos, életet veszélyeztető cardiovascularis szövődmények megelőzéséhez. | We present the key aspects of recognizing and managing pheochromocytoma through the case of a young female patient. The report focuses on the differential diagnosis of secondary etiologies underlying severe hypertension and tachycardia in young patients, emphasizing the role of targeted imaging and biochemical testing. Notably, paradoxical blood pressure elevation following betablocker therapy is highlighted as a critical warning sign of an unrecognized pheochromocytoma. Furthermore, we discuss the primacy of alphaadrenergic blockade, the necessity of perioperative hemodynamic optimization, and the role of definitive surgical intervention. Ultimately, this case underscores that early recognition and the correct therapeutic sequence are essential to prevent severe, lifethreatening cardiovascular complications.
