Abstract

A gastroschisis az elülső hasfal veleszületett fejlődési rendellenessége, amely a belek, ritkábban egyéb hasi szervek extraabdominalis hernializációjával jár. Kifejezett visceroabdominalis téraránytalanság esetén a primer hasfalzárás nem minden esetben kivitelezhető biztonságosan, ezért lépcsőzetes hasfalzárás válhat szükségessé silórendszer alkalmazásával. Az Alexis wound protector/retractor alternatív silórendszerként történő alkalmazását gastroschisis esetén elsőként Kusafuka és mtsai ismertették 2005-ben. Egy 38. gestatiós hétre, praenatalisan diagnosztizált gastroschisissel és intrauterin sorvadás jeleivel született, kis születési súlyú fiú újszülött esetét ismertetjük. A hypoplasiás hasüreg és az eventerálódott bélkacsok állapota miatt primer hasfalzárás nem volt kivitelezhető. A hasfali hiány meghosszabbítását követően XS méretű Alexis O wound protector/retractor gyűrűt alkalmaztunk felfüggesztett alternatív silórendszerként. A hernializálódott szervek fokozatos repozíciója napi sebészeti kontroll és rendszeres silószűkítés mellett történt. A definitív hasfalzárásra a posztoperatív 10. napon került sor, folyamatos intravesicalis és intraabdominalis nyomásmonitorozással, 10–11 cmH 2 O nyomásértékek mellett. A posztoperatív szak eseménytelen volt. Tapasztalataink alapján az Alexis O wound protector/retractor gyűrű biztonságos, szövetbarát és költséghatékony alternatívát jelenthet a hagyományos silórendszerekkel szemben a gastroschisis lépcsőzetes kezelésében. Szerkezeti tulajdonságai, széles méretválasztéka és felfüggeszthetősége alkalmassá teszi újszülöttsebészeti és intenzív terápiás alkalmazásra. Esetünkben az eszköz megfelelő ideiglenes hasfali fedést biztosított, lehetővé tette a hernializálódott szervek fokozatos repozícióját, támogatta az infekciókontrollt, valamint hozzájárult a sikeres halasztott hasfalzáráshoz. Orv Hetil. 2026; 167(39): 1574–1577. Gastroschisis is a congenital abdominal wall defect associated with extraabdominal herniation of the bowels and, rarely, other abdominal organs. In cases of severe visceroabdominal disproportion, primary abdominal wall closure may not be feasible, therefore staged reduction using silo systems is required. The use of the Alexis wound protector/ retractor as an alternative silo system in gastroschisis was first described by Kusafuka et al. in 2005. We present the case of a restricted neonate born at 38 weeks of gestation with prenatally diagnosed gastroschisis. Due to the hypoplastic abdominal cavity and the condition of the eventerated bowel loops, primary closure could not be performed. Following abdominal wall defect enlargement, an XS-sized Alexis O wound protector/retractor ring was applied as a suspended alternative silo system. Gradual reduction of the herniated organs was achieved during daily surgical care and regular silo narrowing. Definitive abdominal wall closure was performed on postoperative day 10 under continuous intraabdominal pressure monitoring at 10–11 cmH2O. Alexis O wound retractor proved to be a safe, tissuefriendly and cost effective alternative to conventional silo systems in the staged treatment of gastroschisis. Its structural properties, wide size range and the possibility of suspended fixation make it suitable for neonatal surgical and intensive care use. In our case, the device provided effective temporary abdominal coverage, allowed gradual reduction of the herniated organs, supported infection control and contributed to successful delayed abdominal wall closure.
