Abstract
| The management of urinary stone disease has evolved substantially with the development of flexible ureterorenoscopy and advanced laser lithotripsy technologies. Holmium:YAG, Thulium:YAG, and thulium fiber lasers have different physical characteristics, enabling a wide range of stone treatment strategies. Appropriate laser settings should be selected according to stone composition, size, anatomical location, as well as the characteristics of the laser system and endoscopic equipment used. In general, fragmentation is performed using higher pulse energy and lower frequency, whereas dusting relies on lower energy and higher frequency. Popcorning and pop-dusting techniques can facilitate further fragmentation and clearance of residual stone fragments. Particular attention should be paid to the balance between laser power and irrigation in order to minimize thermal injury and excessive intrarenal pressure. Stone composition and anatomical conditions may substantially influence the effectiveness of laser lithotripsy. Individualized selection of laser parameters and treatment strategy based on stone characteristics, anatomy, and available technology may improve the efficacy and safety of endoscopic stone treatment.
