Abstract
| Introduction: At the time of diagnosis of clear cell renal cell carcinoma, nearly one-third of patients already have confirmed distant metastases. In metastatic disease, cure is not achievable with the treatment options currently available, systemic and surgical treatment aims to improve the patient’s life expectancy and quality of life. Case report: The patient had an 8 cm right-sided clear cell renal cell carcinoma with multiple liver and lung metastases, with a thrombus extending into the renal vein and 4 cm into the vena cava. Combined immunotherapy (nivolumab+ipilimumab) resulted in regression of the metastases. Histopathological examination following delayed nephrectomy revealed no detectable tumor cells. After additional nivolumab treatment, further regression of distant metastases was confirmed. Two years after diagnosis, no metastasis can be detected. Conclusion: With our case report, we wanted to draw attention to the fact that in certain cases, the available systemic treatment options (with particular regard to immunotherapy) provide surprisingly long survival for our patients. In the future, it will be important to identify which patients are likely to achieve such prolonged survival benefit and for whom it is worth accepting the adverse drug reactions and costs of immunotherapy.
