Abstract
A Peyronie-betegség a hímvessző szerzett, fibrotikus megbetegedése, amely péniszdeformitáshoz, szexuális funkciózavarhoz és jelentős pszichoszociális terheléshez vezethet. A betegség konzervatív kezelése továbbra is kihívást jelent, mivel az elérhető terápiás lehetőségek evidenciaszintje jelentős heterogenitást mutat. Jelen közlemény célja a Peyronie-betegség konzervatív kezelési lehetőségeinek áttekintése a rendelkezésre álló tudományos evidenciák és nemzetközi irányelvek tükrében, különös tekintettel a mindennapi klinikai gyakorlatban alkalmazott terápiák kritikai értékelésére. A konzervatív kezelések elsősorban az aktív fázisban alkalmazhatók a fájdalom csökkentésére és a progresszió mérséklésére. Stabil fázisban a collagenase clostridium histolyticum rendelkezik a legerősebb evidenciával a görbület csökkentésében. A napi adagolású PDE5-gátlók elsősorban a társuló erectilis diszfunkció kezelésén és a funkcionális állapot javításán keresztül játszhatnak szerepet mind az aktív, mind a krónikus fázisban. A hialuronsav-alapú kezelések ígéretes eredményeket mutatnak, ugyanakkor evidenciaszintjük jelenleg korlátozott. Számos más, korábban alkalmazott kezelés nem rendelkezik megfelelő bizonyítékkal, ezért rutinszerű alkalmazásuk nem javasolt. A Peyronie-betegség kezelése során alapvető a bizonyítékokon alapuló, betegközpontú megközelítés, amely figyelembe veszi a betegség fázisát, a deformitás mértékét, valamint a társuló funkcionális és pszichoszociális tényezőket. | Peyronie’s disease is an acquired fibrotic disorder of the penis that may result in penile deformity, sexual dysfunction, and significant psychosocial burden. Conservative management of the disease remains challenging due to the heterogeneity in the level of evidence supporting currently available treatment options. The aim of this review was to summarize current conservative treatment strategies for Peyronie’s disease in light of available scientific evidence and international guidelines, with particular emphasis on therapies used in everyday clinical practice. Conservative treatments are primarily indicated in the acute phase to relieve pain and limit disease progression. Collagenase clostridium histolyticum has the strongest evidence for reducing penile curvature in the stable phase. Daily low-dose phosphodiesterase type 5 inhibitors may improve functional outcomes through the treatment of associated erectile dysfunction in both acute and chronic phases. Hyaluronic acid-based therapies have shown promising results, although their level of evidence remains limited. Several other previously used therapies lack sufficient evidence and, therefore, they are not recommended for routine clinical use. An evidence-based and patient-centered approach is essential in the conservative management of Peyronie’s disease, taking into account disease phase, severity of deformity, and associated functional and psychosocial factors.
