Abstract
| IgG4-related disease is a chronic systemic autoimmune condition that may involve multiple organs; however, pituitary involvement is rare. The authors present the case of a 42-year-old male patient who was evaluated due to headache, significant weight loss and symptoms of hypopituitarism. Laboratory findings confirmed central hypothyroidism, hypogonadotropic hypogonadism and central adrenal insufficiency. Cranial CT and sellar MRI demonstrated thickening of the pituitary stalk and compression of the optic chiasm. Histopathological examination revealed lymphoplasmacytic infiltration with increased numbers of IgG4-positive plasma cells, suggesting IgG4-related hypophysitis. Transnasal pituitary surgery did not result in remission, and the patient’s condition progressed with the development of diabetes insipidus and visual field impairment. Highdose corticosteroid therapy led to temporary improvement; however, due to further progression, stereotactic radiotherapy was required. The patient is currently in stable condition under hormone replacement therapy. IgG4- related hypophysitis is a rare condition that poses diagnostic and therapeutic challenges, requiring a multidisciplinary approach and long-term follow-up.
