Abstract

In chronic total occlusions (CTOs) percutaneous coronary intervention (PCI) balloon uncrossable lesions are relatively common, these are lesions which cannot be crossed with a balloon or a microcatheter after successful intraluminal advancement of the guidewire distal to the lesion. Laser atherectomy could assist in the treatment of such lesions, especially because it does not require a dedicated guidewire compared to rotational atherectomy. In this report we share the first Hungarian experience about laser atherectomy of coronaries, which is widely used in other fields of medicine. In the first case the laser successfully crossed a balloon uncrossable lesion, while in the second case the laser assisted the rotational atherectomy. Both cases resulted in successful CTO recanalization without any procedural complications. | A krónikus koronária-okklúziók (CTO) katéteres rekanalizációja során viszonylag gyakran előforduló akadály, hogy a disztálisan intraluminalis pozícióba jutott vezetődrótra nem tudunk mikrokatétert vagy monorail ballont vezetni, és ez végső soron a beavatkozás kudarcát okozza. A lézer angioplasztika ezekben az esetekben segítségünkre lehet különös tekintettel arra, hogy a rotációs atherectomiával szemben itt nincs szükség speciális vezetődrótra. Esetismertetésünkkel az orvostudományban egyéb területeken már széles körben alkalmazott lézertechnika ilyen irányú felhasználásának első hazai tapasztalatait szeretnénk megosztani. Az első esetben korábban sikertelen, ballonnal áthatolhatatlan elzáródáson sikeresen jutottunk át lézerkatéterrel, míg második esetünkben lézerrel módosítottuk a plakkot, amely megteremtette az eredményes rotabláció feltételét. Végül mindkét esetben sikeres, szövődménymentes rekanalizáció történt.
