Abstract

Az uroszepszis húgyúti fertőzés talaján kialakuló, életet veszélyeztető, szervdiszfunkcióval járó állapot. A korai felismerés, valamint az időben megkezdett empirikus antibiotikum- és folyadékterápia alapvetően meghatározza a kimenetelt. A közlemény célja az uroszepszis aktuális diagnosztikai és terápiás ajánlásainak áttekintése a 2026-os Surviving Sepsis Campaign irányelvei alapján. Bemutatja a szepszis korai felismerését segítő „early warning” pontrendszerek szerepét, valamint a diagnosztika és az antibiotikumterápia aktuális ajánlásait. Az irányelv egyik legfontosabb újítása, hogy az antibiotikumterápia időzítését nem egységesen, hanem a szepszis valószínűsége és az állapot súlyossága alapján határozza meg. A jelenlegi ajánlások célja a korai, célzott kezelés elősegítése, miközben mérsékelik a szükségtelen antibiotikumhasználattal összefüggő kockázatokat. | Urosepsis is a life-threatening condition associated with organ dysfunction developing secondary to urinary tract infection. Early recognition and timely initiation of empiric antimicrobial and fluid therapy are key determinants of outcome. The aim of this review is to summarize current diagnostic and therapeutic recommendations for urosepsis based on the 2026 Surviving Sepsis Campaign guidelines. It reviews the role of early warning scoring systems in the early recognition of sepsis, as well as current recommendations regarding diagnostics and antibiotic therapy. One of the most important innovations of the guideline is that the timing of antibiotic therapy is determined not uniformly, but according to the probability of sepsis and the severity of the patient’s condition. Current recommendations aim to support early, targeted treatment while reducing the risks associated with unnecessary antibiotic use.
