Abstract
A laphámrák lovakban gyakran előforduló rosszindulatú daganat. Klinikailag leggyakrabban a szemtájékon, valamint a genitáliák környékén jelenik meg. Jelen tanulmányban a szerzők egy praeputialisan, valamint a péniszen megjelenő laphámrák sikeres kezelését mutatják be egy herélt lovon. Az elváltozások 2019-ben jelentek meg, majd a lovat 2021 márciusában kezelték a szerzők. A kezelés során többféle módszert kombinálva jutottak el az elváltozás kórszövettanilag is igazolt gyógyulásáig. Alkalmazott módszerek: sebészi kimetszés elektrokemoterápiával kombinálva, elektrokemoterápia, kemoterápiás kezelések (lokális valamint szisztémás formában). | Background: Squamous cell carcinoma is a commonly occurring malignant tumor in horses, frequently found in the ocular region and genital area. The tumor primarily appears on non-pigmented skin, especially in areas exposed to UV radiation. The development of SCC may be associated with the increased production of smegma. Objectives: This study presents the successful treatment of squamous cell carcinoma located on the prepuce and penis of a gelding. The treatment approach involved a combination of multiple methods, leading to histologically confirmed recovery of the lesions. The methods applied included surgical excision combined with electrochemotherapy, standalone electrochemotherapy, and chemotherapy treatments (both local and systemic). Material and Methods: Following the inquiry of the owner an initial examination was conducted, during which squamous cell carcinoma (SCC) was suspected based on the physical findings. A biopsy was performed and the results confirmed a grade II, moderately differentiated, ulcerative squamous cell carcinoma with necrosis at the margins. Results and Discussion: Following the biopsy results electrosurgery was performed combined with cisplatin infiltration and electrochemotherapy. A second round of cisplatin infiltration and electrochemotherapy was performed for the remaining marginal lesions which was continued with 5-fluoro-uracil emulsion to kill the reamining tumor cells. The complete healing was confirmed by a negative biopsy result 9 month later. New lesions appeared on the other side of the prepuce which were treated with carboplatin emulsion. The small remaining lesions were treated with topical 5-fluoro-uracil cream until complete healing. Based on this case, it can be stated that chemotherapy treatments led to bet- ter outcomes than surgical excision alone. However, treatment should always be optimized for the individual case.
