Abstract

A belső fület érintő infarktus, amely elsősorban az arteria cerebellaris anterior inferior egy végágának elzáródásával függ össze, heveny szédüléssel és sensorineuralis hallásvesztéssel jelentkezik. Gyakran a perifériás egyensúlyozó rendszert érintő kórképeket utánoz, a hagyományos képalkotó vizsgálatokon sem mutat kóros eltérést, megnehezítve ezzel a diagnózist. Kezelés hiányában a maradandó egyoldali hallásvesztés jelentősen rontja a betegek életminőségét. A jelen áttekintés a témában újabb tanulmányok eredményeit szintetizálja a kórkép klinikai jellemzőivel, diagnosztikájával és terápiás megközelítéseivel kapcsolatban. A betegek elhúzódó szédüléssel (>24 óra), egyoldali halláscsökkenéssel és fülzúgással jelentkeznek. Vizsgálatkor perifériás eltérésre utaló tünetek figyelhetők meg (harmonikus vestibularis tünetegyüttes, pozitív fejimpulzusteszt), miközben a centrális eredetre utaló egyetlen jel a hallásvesztés, amely gyakran észrevétlen marad, sokszor a betegek sem veszik észre. A vascularis társbetegségek – mint a magas vérnyomás és a cukorbetegség – jelentenek rizikófaktorokat. A diagnosztikai nehézségek az ismeretlen eredetű hirtelen sensorineuralis hallásvesztéssel és a Ménière-betegséggel való átfedésből adódnak; a vascularis eredet a nagy rizikójú populációkban az akut egyensúlyi és hallórendszeri szindrómák akár 15–20%-áért felelős lehet. A korai thrombolysis vagy thrombocytaaggregáció-gátló kezelés javíthatja a kimenetelt, thrombolysis azonban ritkán történik a képalkotó jelek hiánya miatt. A diagnózis megszületéséhez fontos akutan a HINTS Plusz, a későbbiekben a videós fejimpulzusteszt, a hallásvizsgálat és az MR-képalkotás. A kezelésben kiemelt szerepe van a tartós szédülés esetén alkalmazott egyensúlyi és hallásrehabilitációnak, valamint a másodlagos stroke-prevenciónak. Összefoglaló tanulmányunk rámutat a vestibulocochlearis stroke aluldiagnosztizáltságára, és a szédüléssel-hallásvesztéssel jelentkező betegek integrált neurootológiai és vascularis kivizsgálását szorgalmazza protokoll alapján a gyorsabb és hatékonyabb kezelés, valamint a későbbiekben nagyobb eséllyel kialakuló nagyagyi stroke elkerülésének irányába tett prevenciós lépések érdekében. Orv Hetil. 2026; 167(33): 1309–1317. | Vestibulocochlear infarct, most commonly associated with infarction of a terminal branch of the anterior inferior cerebellar artery, presents with acute vertigo and sensorineural hearing loss. It often mimics peripheral vestibular disorders and may show no abnormal findings on neuroimaging, making the diagnosis particularly challenging. Without appropriate treatment, permanent unilateral hearing loss can significantly impair patients’ quality of life. This review synthesizes findings from recent studies regarding the clinical characteristics, diagnostic approach, and therapeutic strategies of this condition. Patients typically present with prolonged vertigo (>24 hours), unilateral hearing loss, tinnitus, and imbalance. On examination, peripheral signs such as a positive head impulse test or horizontalrotatory nystagmus may be observed, while the only indicator of a central origin is the hearing loss, often subtle or unnoticed even by the patient. Vascular comorbidities, including hypertension and diabetes mellitus, constitute key risk factors. Diagnostic difficulties arise from overlap with idiopathic sudden sensorineural hearing loss and vestibular neuritis; a vascular origin may account for up to 15–20% of acute audio-vestibular syndromes in high-risk populations. Early thrombolysis or antiplatelet therapy may improve outcomes, though thrombolysis is rarely administered due to the lack of imaging evidence. For diagnosis, video head impulse testing, audiometry, and MR neuroimaging are essential. Management relies on vestibular and auditory rehabilitation for persistent vertigo and secondary stroke prevention. Our review highlights the underdiagnosis of vestibulocochlear stroke and advocates for an integrated neuro-otological and vascular diagnostic protocol in patients presenting with vertigo and hearing loss to facilitate faster and more effective treatment.
