Abstract

A rituximab, amely a B-sejtek CD20 felszíni antigénjéhezkötődő monoklonális antitest, az utóbbi években egyre jelentősebb szerepetnyert el az immunmediált glomerulonephritisek terápiájában. Első vonalbeli terápiává vált az ANCA-asszociált vasculitisek remisszióindukciós kezelésében, ésjavasolt a betegségben fenntartó immunszuppresszióként is. Indikációja lupusnephritisben korlátozódott, szerepét ebben a betegcsoportban más biológiai terápiás készítmények veszik át. Közleményünkben összefoglaljuk a rituximab fentikórképekben történő alkalmazásának indikációit, a potenciális mellékhatásokat,a terápiarefrakter esetek patomechanizmusát, röviden hivatkozva a két kórképkomplex immunszuppresszív és alternatív kezelési lehetőségeire is. Az összefoglaló szakmai hátterét a KDIGO 2024 Clinical Practice Guideline for the Management of Antineutrophil Cytoplasmic Antibody (ANCA)-Associated Vasculitis, azEULAR 2022 és az ERA Immunonephrology Working Group 2023 szakmai irányelvei és további kontrollált vizsgálati eredmények képezik. | Rituximab, a monoclonal antibody that binds to the CD20 surface antigen of B cells, has gained an increasingly important role in the therapy of immune-mediated glomerulonephritis in recent years. It has become a first-line therapy for the induction of remission in ANCA-associated vasculitis and is also recommended as a maintenance immunosuppression in the disease. Its indication was limited to lupus nephritis, and its role in this patient group is being taken over by other biological therapeutic agents. In our publication, we summarize the indications for the use of rituximab in the above diseases, its potential side effects and the pathomechanism of therapy-refractory cases, briefly referring to the complex immunosuppressive and alternative therapies of the two diseases. The professional background of the summary is the KDIGO 2024 Clinical Practice Guideline for the Management of Antineutrophil Cytoplasmic Antibody (ANCA)- Associated Vasculitis, the EULAR 2022 and ERA Immunonephrology Working Group 2023 professional guidelines, and additional randomized controlled trial results.
