Abstract

A húgyúti kövesség recidívájának megelőzése a kő összetételének pontos meghatározásán, a beteg rizikóbesorolásán és a metabolikus eltérések célzott kezelésén alapul. A magas kockázatú és recidív kőképzők esetében két, egymást követő, 24 órás vizeletgyűjtésen alapuló metabolikus kivizsgálás teszi lehetővé a fenotípusorientált metafilaxis kialakítását. Húgysavkövességben a tartósan alacsony vizelet-pH korrekciója (elsősorban kálium-citráttal) a kezelés meghatározó eleme. Cystinuriában fokozott folyadékbevitel, a nátrium- és fehérjebevitel csökkentése, alkalizálás, szükség esetén tiopronin alkalmazása indokolt. Infekciós kövekben a teljes kőmentesítés és a perzisztáló infekció megszüntetése alapvető. A célzott metafilaxis a recidívák és a kapcsolódó vesefunkció-romlás kockázatának csökkentését szolgálja. | Prevention of urinary stone recurrence is based on stone composition analysis, individual risk stratification, and targeted treatment of underlying metabolic abnormalities. In high-risk and recurrent stone formers, two consecutive 24-hour urine collections provide the basis for a metabolic evaluation and individualized metaphylaxis. For calcium-containing stones, adequate fluid intake, normal dietary calcium intake, and restriction of sodium and animal protein represent the cornerstone of preventive management. Thiazide or thiazide-like diuretics may be used in hypercalciuria, potassium citrate in hypocitraturia, dietary oxalate restriction in confirmed hyperoxaluria, and dietary purine restriction with allopurinol when hyperuricosuria is present. In calcium phosphate stone disease, primary hyperparathyroidism, distal renal tubular acidosis, and urinary tract infection should be excluded. In uric acid stone disease, persistently low urinary pH is often the major pathogenic factor; therefore, urinary alkalization with potassium citrate is the mainstay of treatment and may enable oral chemolysis. Cystine stones require intensive fluid intake, sodium and animal protein restriction, urinary alkalization, and, in selected patients, tiopronin therapy. In infection-related stones, complete stone clearance and eradication of persistent urinary infection are essential to eliminate the infectious nidus. Individualized metabolic metaphylaxis may reduce stone recurrence and help prevent progressive renal damage associated with recurrent obstruction and infection.
