Abstract

Bevezetés: Az esettanulmány a szinkron urogenitalis daganatok diagnosztikai és terápiás nehézségeit, valamint a modern, szimultán alkalmazott sztereotaxiás sugárkezeléssel (SBRT) kapcsolatos tapasztalatokat mutatja be egy konkrét beteg kórtörténetén keresztül. Esetismertetés: Prosztata-adenokarcinómával diagnosztizált beteg staging kivizsgálása során felfedeztünk egy, a jobb vesében, centrálisan elhelyezkedő, 20 mm átmérőjű vesesejtes karcinómát. A páciens számára a műtét magas kardiológiai kockázatot jelentett, emiatt az onkoteam mindkét daganat esetén sztereotaxiás besugárzást javasolt, a prosztatadaganat kezelését androgéndeprivációs terápiával kiegészítve. A szisztémás progresszió kontrollálása érdekében jelenleg másodvonalbeli hormonkezelés beállítása zajlik. Következtetés: A magas műtéti kockázatú betegek szinkron urogenitalis daganatai esetén a szimultán alkalmazott sztereotaxiás sugárkezelés a radikális sebészet hatékony és biztonságos alternatívája lehet. | Introduction: This case study presents the diagnostic and therapeutic challenges of synchronous urogenital tumors, as well as experiences with modern, simultaneously applied stereotactic body radiotherapy (SBRT). Case Report: A patient diagnosed with prostate adenocarcinoma was found to have a concurrent 20 mm renal cell carcinoma located centrally in the right kidney. Due to the patient’s concomitant cardiac condition the surgical approach posed a high risk. Therefore, the multidisciplinary tumor board opted for stereotactic radiotherapy for both malignancies, supplementing the prostate cancer treatment with androgen deprivation therapy (ADT). As a result of the combined treatment, the disease remained in complete remission for two years. However, in 2025 progression of the prostate cancer occurred; in addition to an elevated PSA level, a solitary metastasis developed on the third lumbar vertebra. To control the systemic progression, the initiation of second-line hormonal therapy is currently underway. Conclusion: In cases of synchronous urogenital tumors in patients with high surgical risk, simultaneously applied stereotactic radiotherapy can be an effective and safe alternative to radical surgery.
