Abstract
Kivizsgálásunk során a felmerülő gyakori urológiai kórképeket kizártuk. MRI-vizsgálat a bal oldali vese agenesiája mellett azonos oldali maradványurétert mutatott. Az urétercsonkot eltávolítottuk. A szövettan malignitást nem igazolt. Preoperatív képalkotó vizsgálat felvetette apró vesicovaginalis fisztula (VVF) lehetőségét, amelynek tüneteit a beteg műtét előtt nem mutatta, azonban posztoperatív időszakban vizeletcsorgást észlelt a hüvely felől, így halasztottan, Latzko szerinti transvaginalis fisztulakiirtást és -zárást végeztünk. Ezután a beteg tünetmentessé vált. Esetünk ritkasága ellenére bizonyítja, hogy hétköznapinak imponáló kismedencei fájdalom hátterében is felmerülhet ritka fejlődési rendellenesség, amely mégis komoly tüneteket és életminőség-romlást okozhat a betegnek. | The examination of a 59-year-old female patient began at our clinic due to lower abdominal pain and nausea. No infectious urological, gastroenterological, or gynecological causes were found in the background of her complaints. During our examination, common urological disorders were ruled out. Magnetic resonance imaging (MRI) scan confirmed left renal agenesis and a remnant ureter on the ipsilateral side. However, this syndrome was not confirmed in this case. We removed the ureter stump. Histology did not confirm malignancy. Preoperative imaging suggested the possibility of a small vesicovaginal fistula (VVF). Nevertheless, the patient did not show symptoms of this condition before surgery, but postoperatively, urine leakage was observed from the vagina. This led to a delayed transvaginal fistula excision and closure according to Latzko. After that, the patient became symptom-free. The rarity of our case proves that even in the presence of seemingly common pelvic pain, a rare congenital disorder may underlie the clinical manifestations, and can cause serious symptoms and deterioration in the patient’s quality of life.
